Hello, I am a couples therapist and my website is www.mainlinecounselingpartners.com. I would like to be able to post questionnaires up where people can go to my site, answer a series of questions which would then be emailed backt o me. Is there a plugin for something like this? I have contact form 7 working well, people can fill in their name and address but I don't think it can collate data. Thanks, Laura
