Go to Appearance > Widget Drag "Ink Appointment Widget"and drop it in the Primary widget area, This will solve your issue.
That was a little helpful, although I was looking for the Appointment, Business Hours, and Location to copy over. Also I would like the submit button within the Ink Theme Appointment Request to generate an email with the gathered information. It can still send it to the CSV file as well (I don't really care about that piece) but I need the email generation.
