Adding more Fields in Appointway widget

Going straight...

I have two (2) questions:

1.)
Is it possible that I can add more fields from the default appointway widget?
from default fields: Name, Contact Number, Email.

I want to add Department, City/State, Rank/Title and Address field..(just a static text)
Please advice.

2.)
How can I change the font color for all pages and also the page tittle? Because as you can see we are using the Black background (which is image type).
I am planning to change the background in css code to make it less heavier.

1. Unfortunately there isn't a way to do that with our theme unless you custom code something up. Sorry.
2.
Paste this code in Custom CSS
Appearance > Theme Option > Styling Option > Custom CSS
