Adding Opt-in Subscriber form in place of Lead Capture Form

I would like to replace the Lead Capture form that is displayed on both my Home Page and inner Pages/Posts, with my own MailChimp Email Susbscriber Opt-in form.

I understand I will need to have this Custom coded but can you please let me know which all the files I will need to modify in order to get my Mailchimp for to work.

You have change form used in sidebar from your widget area.Remove lead capture form and use MailChimp widget.

When I remove the lead Capture widget the lead capture form gets removed from all the inner pages but it does not get removed from the Home Page. My website is setup with the configuration Front Page 'On'.
If you look at my website mavendomains(dot)com you will still see the Lead Capture form on the Home Page. How do I replace the home page Lead Capture form with my MailChimp form?
