Appointment form on Page

Is it possible to have the appointment form on a page? I want to add the form to a menu item as well as sidebar.

Hello,

It is not possible to add the appointment form on a page.
But you can display the appointment form in sidebar of the pages.
For this Go to the Appearance > Widgets panel in your dashboard and
then drag & drop the "Ink Appointment Widget" from Available Widgets section in the Primary or Second Widget Area.

This will display the appointment form in sidebar of the pages.
