Appointments Form gives after completion "Please insert All data"

    Hi,

    When I complete the appointment form on my website ( http://www.pedicure-tilburg.nl )
    I get the following message:"Please Insert All data."

    Ofcourse I have filled-out the form completely, can you help or give suggestion how to solve this.

    I'm running Appointment way Pro theme V1.1.8 with Ink Appointment Pro Plugin V1.2.0

    I think you have not configured "Booking appointment" settings properly.
