Appointway form styling not matching frontpage styling

Hi,

First, I am wondering why Appointway 1.05 is not triggering an update in Wordpress. I ask because my form appearing on front page, while fine, does not render on other pages the same.

The front page has this code that I assume renders the desired look:
<div class="grid_8 omega ipad"><div class="feature_sidebar"><div class="appointment_wrapper"><div class="appointment_form_top"></div><div class="appointment_form_wrapper">

The other pages only have this code:<div class="grid_7 omega"><!--Start Sidebar--><div class="sidebar blog">

How can I get my other pages to insert the former code to render the look I desire?
