Can't Add Field to Lead Capture Forum

Everytime I try to add a field to the lead capture forum, nothing happens.

Please help

 

Also, I deactivated my plugins and this did not help.

Please install a fresh copy of theme if issue is not resolved.
We have checked and fields created correctly.

How do I do that? Do I need to delete the current theme? Will this remove all of my data and settings?

We have checked your site and find that you have set some security through wp security plugins or any another issues may be of your hosting.
This is not allowed to read jquery or edit files of jquery through wp editor.
