Hello, It is not possible to add new fields and change the fields in the homepage lead capture form, But you can rename the field of the homepage lead capture form, if you want to do so then, Go to functions > localbusiness-leads-form.php present in your theme directory and follow the instruction as shown in image given below Thanks & Regards Gourav Shrivastava
