Custom Leads Form Issues

The Custom Leads Form on my site was working fine one day now the only thing that will appear is the Captcha Field and I can't add anything else to the form. Not sure what I've done or how to correct this. http://carmineweb.logicalcustomsolutions.com

I have checked your website it seem that you have removed the Lead capture form from the website.
So please add Lead capture form in your website and then let us inform.

 

I have re-added the Lead Capture Form to the web site.

Download updated version of theme from your members area and upload it, This will solve your issue.

I downloaded and installed the updated version as per your instructions but now I get an Input error on the form and I still cannot add any fields to the form.

Hello,

Go to Appearance > Theme Options > Contact Form Recaptcha Settings> Recaptcha Public Key/Recaptcha Private Key
and enter keys there.

This will solve your issue.
