Having issues customising the appointment form default on the home page. Don't know where or how to customise it. I started created a custom lead form but everytime i use the field name as "suburb" i get 3 showing up on the homepage. See image 1 for explanation. Second image shows where i am trying to get. I'd like to customise some of the field in the existing default form. I'd like to add new fields to the default form and i'd like to place a little note somewhere on the form in smaller fonts. Any help will be appreciated.
Hello, For lead capture form. Please update your theme from members area. For appointment form. 1) You can change form headings from Dashboard > Appointment > Settings 2) Adding extra fields require customization in the apt-form.php file. 3) To add extra text go to the file address given below and add text as shown in the image attached. plugins\ink-appointment\ink-admin\apt-form.php Thanks & Regards Nitesh Raghuwanshi
