Default Contact Form Problems

When trying to use the default contact form, I have a number of issues.

1.) When there's an error, red text is displayed, however focus jumps back to the top of the page.
2.) When Captcha is turned on, error invalided referer is display near submit button, not the captcha box.
3.) How can we add more fields to the default form? Can i safely modify the contact.php form?

4.) from a related post, i'd much rather use the Contact Form 7 plugin here, but can't use short codes.
