eMAIL Contact Us Form

Can this be edited?

The email that I [the admin] receives [to my outlook] from the Contact Us form [from the site] is shown below.

Can the "FROM" field - that currently shows the visitors name - be edited to show the SITE NAME? or also shows the site name for example:

[SITE ABC] Mary

OR

Instead of showing [PHP Snippets] under subject line - can it show the SITE name. (THIS option will probably the best one)
