Hello, That is the default form which is added just for the demo purpose. If you want to use same form, then go to the FormGet Website (Login from here: http://www.formget.com/ ) and create your form and then copy the embed code from there and paste it in the text widget of Home Page Sidebar Feature Widget Area.
