How do you set up the middle categories just like your preview site?

Hi, I don't know how to set something like this (Screen Shot 2013-04-11 at 11.43.28 PM) to like your live preview example: (Screen Shot 2013-04-11 at 11.45.07 PM)

Can you come up with an online video tutorial on youTube, please? so that I can learn.
