Hello,

That is the default form which is added just for the demo purpose.
If you want to use same form, then go to the FormGet Website (Login from here: http://www.formget.com/ ) and
create your form and then copy the embed code from there and paste it in the text widget of Home Page Sidebar Feature Widget Area.

You are free to create your form but you can only use its free features and there are limits on entries in form i.e. 300.
