how to link the optin form button to a thank you page

Hi There, I am trying to add a link to a thank you page when some opts in using the form on the home page. How do I do this? The thank you is made using lead pages...Thanks, Sandra

From your above words, It seems that you want to create a link of your lead form to the thank you page means if someone submit the form through homepage then that would be diverted to the thank you page..
