How to remove default contact info from contact form

Right now the contact form populates with the website owners contact info. Can you please tell me how to remove that so that all the contact fields are blank?

This is not our theme default contact form.
I think you are using any plugin to display contact form.
So contact with your plugin related support form they will help.
