I have installed the Lead Capture Pro plugin and created a form with 5 fields, consisting of name, email, phone, address, and message. I'm using the Lead Capture widget in a sidebar and I've encountered 3 issues (see attached jpg) -- and need some expert help to resolve them. (1) I would like to have the address field appear after the phone field and before the message field -- I tried deleting the message field and then adding it after adding the address field. However, address field keeps appearing after the message field. I'm guessing that there is some editing needed in the leads-customform.php file. (2) The message text field appears to extend beyond the form container. Looks like there is CSS type issue, possibly in the leads-custom-form.css file. (3) There are some bullets/dots on the left-hand side of the form, which I would like to remove. This probably requires some more CSS editing. Would greatly appreciate your providing guidance on fixing the above 3 issues. Thank you

I tried deactivating all plugins except for the Lead Capture Pro plugin. However, the form in sidebar is still "hosed". Also tried installing the Lead Capture Pro plugin in a different premium theme (i.e., ElegantThemes Divi 2.0) -- and it WORKED perfectly. This tells me that the cause of the problem is inherent in the original WP theme that I was using, namely MyThemeShop's Schema WP theme. I have contacted the MyThemeShop support to resolve this problem. Therefore, this ticket can be closed.
