I changed the names of some of the fields and now content is not going where it is supposed to go. I am guessing this is related to the html variable name. Can you please post the original html vsr names so hopefully things will line up again.
Another question, is it necessary for the map longitude and latitude fields be in the form for the map to work? I used only address - and the map did not work.
