Lead Capture Form Not Working After Updating WordPress (LOST ALL LEADS)

    Hello,

    I've just updating my website to the newest version of WordPress. The theme appears to be up to date as well as all of the plugins.

    Immediately after updating I checked my website and notice that all of the fields for the lead capture form are gone except for the 'submit' button.

    I updated because the leads capture form was no longer delivering leads to my email inbox, but only to the leads capture database in my dashboard.

    When viewing the page source, the fields for the leads capture form do not exist in source code either.

    Something in incompatible with the new version of WordPress - there's an issue with the theme.

    Info sent and payment made. Hoping for a resolution.

    This is what is NOT working:
    - All leads have disappeared
    - Leads capture form on the homepage does not show any input fields other than submit (which we have named "request free quote")
    - I cannot create new fields in the leads capture form setup... no fields save when I try to create them and all old fields are gone
    - Leads submitted make it into the leads capture page within WordPress, but are never emailed to me... regardless of how I format the email address in settings.
