Lead Capture Plugin: How to remove unwanted lines of text.

Hello ... again.

I've added InkTheme's Lead Capture plugin to a Road Fighter site that I'm working on for a client: http://demo77.digitalwordpower.com/

I've placed the plugin in the Primary Widget Area and it's showing in that area. All is well except for three(3) extraneous lines of text that I want to remove. I had attempted to add a check box or radio option multiple times without success when I set it up. Those attempted entries didn't reflect in the lead capture form set-up, but do show up in this weird way on the displayed form. [See attached screenshot.]

I can't seem to locate any function to delete them.

Any advice? Once this is set up properly, I think it will be exactly what my client is looking for.

Sorry for the delayed reply...

There is a bug in the Lead Capture Plugin regarding radio button on which our developer team is working.
We will upload the updated version of the plugin in the members area as soon as the bug is fixed.

My client is ready to launch this week (tomorrow or the next day ... Thursday or Friday | July 16 or 17). What's the status of the bug fix with this plugin?

You will be glad to know that we have fixed the bug in the latest and updated version of Lead Capture Plugin.
Also, you will be able to create checkbox and radio button in the form.

Praveen, thanks for the update.

I downloaded -- what I thought would be -- the updated version of Lead Capture Plugin, deleted the previous one, and replaced it with the new. However, there seems to be no change.

The three unwanted lines remain...

... even after deleting and attempting to replace all of the other Field Names.

In addition, I can't seem to format the Field Names in the order I would prefer. I must be missing something. Please advise. Thanks for your help.
