It seems the leadform generator is producing multiple entires of Name, Email, etc (see attached snapshot). When looking in the leadform 'custom leads form' admin side there is only one for each however on the front side you can see multiple entires. I can send you a username and password if needed but I do not want to post that information here. Thank you.
