Leads are not showing up in Lead Capture

Hi,

I have the lead capture to switched on but they do not drop in the lead capture dashboard.

I tried it via admin, via a new user and with paid subscription (wont work with free, I know).

Email is send but not with phone number, only name, email and message.

Can someone have a look at what might be the problem?

Where can I send my login info?
