Lost Leads Capture Form & Cannot Rebuild

    Hello,

    I updated my site and the Leads Capture form on the home page slider are was lost.

    I have tried to recreate it in the theme settings but there is an error with the custom form builder. I cannot create fields; when I try to save them, it just resets the page.
