Mailchimp email subscriber form not displaying correctly

    Hi

    I have embedded code copied from my Mailchimp account on to the following page: mavendomains.com/newsletter-subscription

    As you can see the fields are not displaying correctly and the Subscribe button, which is part of the sign-up form, is missing?

    Can you please help me to get this sign-up form working.

    Hello Paul,

    Please use your embed code in right format. You have Subscribe button but it is not displaying so ask to MailChimp support that how should i use this code so that it will display correctly.
