Problem with Lead Capture Form

Hello,

It seems that the Lead Capture Form can't connect to my MySQL DB.

I can't find the reason because the rest of the Site works without any issues.

Please check your username and password of database is correct or not.

Hi,

the password is correct, but maybe it gets lost, can you tell me where i eventually should add it manually?

I could rebuild the failure, and that is what happens.

I had a backup of my site in wordpress 3.*
After i completly deleted my page and set it up new (in the 3.* Version) everything was allright and the form funktion was correct.
After hitting the Update Button to Wordpress 4.0 the form brings the same failure as posted above.

Please download a fresh copy of theme from members area and upload it to your website.
It will resolve your issue.

We have checked our theme works on Wordpress 4.0
