Slider photos not showing / Media Library not showing / Can't edit images

I had not made any changes to this website in months and noticed that the only photo I had in the slider was not appearing. When I tried to edit it in the dashboard (Appearance>Theme Options) it does not allow me to upload or edit the url. It will not save my url change and the button to 'upload' does nothing when clicked. I have not added any additional plug-ins. The entire site was only 2 pages so I uninstalled wordpress and wiped the database and started from scratch and rebuilt the site. I installed Wordpress 4.0 and installed a fresh updated Squirrel theme. It was fine for a few hours today and then this evening, again the photo does not show up, the media library items are gone and I am unable to edit the slider settings.
