"Submit" button in dashboard but does not show up on site page

I have an email list account with Mailchimp to mail out a newsletter monthly that people sign up for onmy website. I have put the HTML code from MailChimp into my page on the dashboard. I see the boxes for name, email address, and "submit" button.
When I view my site their is no submit button. http://marciapell.com/join-my-mailing-list/
Please help me know what to do to correct this issue so people can sign up. Thank you so much!

(Appearance -> Theme Options -> Styling Options -> Custom CSS) of your dashboard.
